Welcome to West Hoe Surgery

All patients with a Cough, Fever or any other respiratory symptoms please inform reception on arrival to the surgery. 

We also offer routine telephone/video consultations in the evening and at weekends  via PushDoctor.

You can get in touch using an Online Consultation (eConsult) and you will be contacted by the surgery within 2 working days of submitting your request. If you need a sick note please request one via eConsult or email/writing to the practice with full information.   

 

New Patients

We accept all patients who live within the Practice area.

Please note you will be asked to complete some forms when you come to the Surgery for the first time. See below for the forms. 

You will also need to bring with you photographic ID such as your passport or driving licence and proof of address such as your Council Tax statement.

You may choose which Doctor or Nurse you would prefer at each appointment.

Our Practice Nurses offer New Patient Checks.

Please note we do NOT accept registration forms via email. Please attend the surgery and pick up the forms or alternatively you can download them but these must be printed and brought into the surgery. 

Each permanent patient needs to complete a GMS1 form and a New Patient Questionnaire.
